Inspector Notes
A self report was received on 10/05/2023 regarding mandated reporting. A staff interview was conducted and documentation was reviewed. A violation was cited regarding mandatory reporting.
A Monitoring Inspection was also conducted today. A total of 3 children (6pts) with the Provider was present. Observation of the home, programming and backyard was made. The children were observed playing with playdoh and interacting with the provider. The children transitioned to the backyard where they took turns riding the tricycles and playing on the playground equipment. A total of 4 children's records, the provider and all household records were reviewed. No medication needs at this time and no identified food allergies. Violations
3Evidence:
1. A brick sidewalk was found with separated bricks. Several lose bricks were laying on the ground presenting a tripping hazard.
2. One child was found attempting to pick up one of the lose bricks.
Evidence:
The door leading to the basement level was open allowing children access to the stairs leading to the basement.
Evidence:
Staff #1 was made aware of a suspected child abuse incident and failed to contact the local department of social services or the Child Abuse and Neglect Hotline.
