Inspector Notes

An unannounced monitoring visit was conducted. Three classrooms were open with 6 direct care staff. All rooms were in ratio for supervision. Observed infant cares, circle time, reading and free play. Reviewed 3 children
s records and 2 staff records, emergency preparation, physical space, emergency preparation and trainings as well as nutrition. Breakfast served today was cheerios and milk with fruit. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 8VAC20-780-350-F
Based on staff statement, the center did not have a written policy to ensure consistency of staff.
Evidence:
No written consistency of staff policy was available following the new Child Day Center Standard updates that went into effect 1/13/2022.
Plan of Correction: A written plan was developed 5/13/2022 and will be shared with staff and new hires.
Standard 8VAC20-780-40-K
Based on center administration statement, a written policy for prevention of shaken baby syndrome and/or abusive head trauma, including safe sleeping practices was not available.
Evidence:
New CDC requirements that went into effect were not written out for SIDS and shaken baby syndrome/abusive head trauma.
Plan of Correction: The written policies were completed 5/13/2022. Staff will be given the written policies as part of new hire orientation.
Standard 8VAC20-780-550-P
Based on record review, injury forms did not contain all required information.
Evidence:
Two injury forms for school year 2021-2022 were reviewed. Missing information was the time the parent was notified and a second signature (parent or staff).
Plan of Correction: Staff will be reminded to complete all information on the minor injury form.

Based on record review, 1 of 3 children's records was incomplete.
Evidence:
Child #1 was missing an updated immunization record every 6 months for a child under two years of age. Last update was 7./2021.
Plan of Correction: The parent will be contacted to bring in a current immunization record.
