Inspector Notes
An unannounced monitoring visit was conducted. Three classrooms were open with 6 direct care staff. All rooms were in ratio for supervision. Observed infant cares, circle time, reading and free play. Reviewed 3 children
s records and 2 staff records, emergency preparation, physical space, emergency preparation and trainings as well as nutrition. Breakfast served today was cheerios and milk with fruit. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. No written consistency of staff policy was available following the new Child Day Center Standard updates that went into effect 1/13/2022.
Evidence:
New CDC requirements that went into effect were not written out for SIDS and shaken baby syndrome/abusive head trauma.
Evidence:
Two injury forms for school year 2021-2022 were reviewed. Missing information was the time the parent was notified and a second signature (parent or staff).
Evidence:
Child #1 was missing an updated immunization record every 6 months for a child under two years of age. Last update was 7./2021.
