Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews (if applicable) and a virtual tour of the program (if applicable).

A renewal inspection was initiated on 12/3/2021 and concluded on 12/6/2021. The director/provider was contacted by telephone and a virtual inspection was conducted. There were 1 child present, with 1 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 1 child record, 1 staff record, and 2 household member records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Evidence: Staff 1, Household Member 1 and Household Member 2 have a central registry search completed on 7/21/2016.
Plan of Correction: Repeat background checks were sent out for all on 12/6/2021.
