Based on record review and staff interview, an orientation training including facility specific topics was not completed for 1 staff member.

Evidence:
1. Staff #1 began employment at this center at the end of January 2024 .
2. The record for Staff #1 did not contain documentation of an orientation training including facility specific topics.
3. Staff stated an orientation specific to the facility was not completed.
Plan of Correction: Director will ensure all employees, to include those from other sites, receive a facility specific orientation within required time frames.
