Inspector Notes

An unannounced monitoring inspection was conducted today from 10:35am-2:00pm. There were 55 children directly supervised by 15 staff. The physical plant, 6 staff records, 5 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed painting and playing with playdough. Handwashing procedures were also observed. Areas of non-compliance are identified in the Violation Notice.

Standard 8VAC20-780-270-A
Based on observation, equipment of the center, inside, was not maintained in a safe condition.

Evidence:
1. Peeling paint was observed in the two's-1 classroom on the wall below the window, accessible to children.
Plan of Correction: A ticket will be put in to request the area be re-painted.
Standard 8VAC20-780-370-5
Based on record review, the child day center did not ensure Infants, who cannot turn themselves over and are awake, shall be placed on their stomachs a total of 30 minutes each day to facilitate upper body strength and to address misshapen head concerns.

Evidence:
1. Documentation in the record for Child #1 indicated "tummy time" occurred at 11:16am on 3/27/24; however a time frame during which tummy time occurred was not documented. The center was unable to provide documentation showing a minimum of 30 minutes of tummy time daily as required.
Plan of Correction: Staff will be re-trained regarding ensuring infants receive at least 30 minutes of tummy time per day, and documenting the time in the record.
