Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 8/29/024 and completed on 8/29/2024. The on-site inspection began at 11:45am and ended at 12:45pm. The inspector reviewed compliance in the areas listed above. There were 8 children present (including a child who resides in the home) and 2 staff. The inspector reviewed 5 children?s records and 3 staff records on-site. This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The provider did not ensure that each child's record contained the telephone number for each custodial parent's place of employment.

Evidence: There were missing work phone numbers for the parents of Child #3 and Child #5.
Plan of Correction: Provider will obtain the work phone number of the parents.
Standard 8VAC20-800-90-C
The family day home did not obtain documentation of additional immunizations for a child, who is not exempt from immunization requirements, once every six months for children under the age of two years.

Evidence: There was no documentation of additional immunizations for a child, who is not exempt from immunization requirements, once every six months for children under the age of two years for Child #1.
Plan of Correction: Provider will obtain an updated immunization record for the child.
