Inspector Notes
An unannounced monitoring/renewal inspection was conducted today between 10am and 11:05am. There were 12 children present with the Provider and 2 assistants for a total of 28 points within the supervision guidelines. The children were observed working on an outer space art project, playing with alphabet letter boards, blocks, and infants were observed receiving bottles. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

2Evidence: Household member #1's most recent TB screening is dated 2/27/2020. The provider confirmed that the household member has not yet completed an updated screening.
Evidence: The thermometer in the family day home refrigerator where children's food and bottles is stored was at 50 degrees.
