Inspector Notes

An unannounced monitoring/renewal inspection was conducted today between 10:40am and 11:25am. There were 8 children present with 2 assistant caregivers within the supervision guidelines. The children were observed doing arts and crafts and playing freely. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Standard 8VAC20-800-140-D
Based on interview, the substitute provider did not record and sign the time of arrivals and departures of each day that the substitute provider worked.
Evidence: At the start of the inspection, caregiver #2 stated that the provider was out of town. Caregiver #2 confirmed that they were not documenting the arrival and departure times that they were working as the substitute provider.

Based on observation, child-resistant protective covers were not used on all unused electrical outlets.
Evidence: There were two uncovered outlets observed on the wall behind the children's high chairs.

Based on record review and interview, documentation of emergency drills was not maintained.
Evidence: There was no emergency evacuation drill documented for the month of March. The provider stated that the drill was completed but they forgot to write it down.
