Inspector Notes

An unannounced code compliance inspection was initiated on 01/10/2024 and concluded on 11:05 a.m. from 11:50 a.m. There were 39 children present, ranging in ages from three to five years old, with 11 staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, procedures, and medication. A total of five children?s records and five staff?s records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on review of record and interview, the center failed to ensure that background check were repeated every five years.
Evidence:
1. Staff #2?s sworn disclosure was dated 11/26/2018. There was no updated sworn disclosure on file for Staff #2.
2. Staff #2 confirmed that the sworn disclosure was not repeated every five years.
Plan of Correction: The sworn statement was completed in the presences of the licensing inspector.
