Inspector Notes

An unannounced Renewal inspection was conducted onsite. A total of 10 children (24pts) with 2 staff were present when the Licensing Inspector arrived. The Provider arrived approximately 30 minutes later. The children were observed in circle time in one area of the home and the younger children were observed being held. A walk thru of the levels of the home was conducted, records were reviewed and staff interviews were completed. A total of 2 cribs are currently being used with the remaining children napping on cots. No medication needs at this time and there were no allergy/sensitivity to food was identified. A total of 3 children's records, all staff and household records were reviewed. Some violations found today. Please email me at: lisa.hudson@doe.virginia.gov with any questions. Based on staff record review, background check requirements were not met.
Evidence:
Staff #2's record did not contain a Sworn Statement, Central Registry Search or a Fingerprint Search on file.
Plan of Correction: Provider will request all required background checks and will forward the results to the Licensing Inspector within 10 days for review.
Standard 8VAC20-800-50-A
Based on review of documentation, the written record of attendance was not up to date.
Evidence:
The written record of attendance was not completed for the following dates: 01/26/22 thru 01/28/22.
Plan of Correction: I will make sure that the attendance is completed every morning at drop off and evening at pick up.
Standard 8VAC20-800-60-A
Based on child record review, some records were incomplete.
Evidence:
1. Child #2's record did not contain a health exam or an immunization record.
Plan of Correction: I obtained both and will maintain them both in the record.
Standard 8VAC20-800-690-A
Based on observation, a child was left unattended on the changing surface.
Evidence:
When the inspector arrived, Staff #2 left Child #1 on the changing surface and walked approximately 12 feet away to open the door for the Licensing Inspector.
Plan of Correction: Discussed the risk of doing this with the staff and that no child will be left unattended on the diaper table.

Based on review of the emergency plan, an annual review was not completed.
Evidence:
The last documented review of the emergency plan was dated January 2020.
Plan of Correction: I will review the plan and note any changes today.
Standard 8VAC20-800-870-G
Based on observation of the family day home, a table was not cleaned after eating.
Evidence:
The table used for eating was not cleaned after breakfast. The Licensing Inspector observed food crumbs on the table surface.
Plan of Correction: Staff were reminded to clean and sanitize the table after each use.

Based on observation of food brought from home, some food was not properly labeled.
Evidence:
1. A can of baby formula was not labeled with the child's name.
Plan of Correction: We will label all food brought from home.
Standard 8VAC20-800-940-C
Based on observation of the family day home and a staff statement, a perishable drink was not refrigerated.
Evidence:
1. A gallon of milk was observed sitting on the counter near the sink.
2. Staff #2 stated that the children like the milk at room temperature.
Plan of Correction: The milk was placed inside the refrigerator and the staff was instructed to always return the milk to the refrigerator.
