Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 04/23/2021 and concluded on 04/27/2021. The Provider was contacted by email to initiate the inspection. There were five children with one caregivers for twelve points which is within ratio for supervision. Reviewed a child's record, medication authorization, physical space (using FaceTIme), trainings and emergency preparation. One medication was reported to be on-site the day of the inspection. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

5Evidence:
No hardware installed gate is present to prevent a child two years of age and under access to the stairs. Child #1 is 11 months of age.
Evidence:
A children's walker was in the daycare space.

Child #2 has an emergency medication on-site and no written authorization from the parent or physician has been completed to keep the medication at the home longer than 10 days.

The medication for Child #2 is not maintained in the original labeled box with identifying information.

A medication for Child #2 expired 7/2020.
