Inspector Notes

A renewal inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and a virtual tour of the Family Day Home (FDH).

This inspection was initiated and concluded on 01/25/2022. The Provider was contacted and a virtual inspection was conducted. There were 9 children present (27 points), ranging from infant to preschool age, with 3 caregivers supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication; special care and emergencies; and nutrition. A total of 9 children records, 1 household record, and 3 staff records were reviewed.

Information gathered during this renewal inspection determined that there were (3) non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the FDH.

Standard 8VAC20-800-210-A
Based on review and interview, a caregiver did not obtain a minimum of (16) sixteen clock hours of training annually in areas relevant to their job responsibilities.

Evidence -
1. The Provider stated (during an interview conducted on the date of inspection - 01/25/2022) that only 1 hour has been completed for Staff #1.
2. This training certificate obtained by Staff #1 was observed to be from an approved online training.
3. Staff #2 and Staff #3's hours of annual training shall be determined after one full year of employment.
Plan of Correction: 16 hours of annual training will be taken by all staff.
Standard 8VAC20-800-90-C
Based on review, 2 of 9 children files indicated that the Family Day Home (FDH) did not obtain documentation of additional immunizations for children (who were not exempt from the immunization requirements according to subsection B of this section) once every six months for children under the age of two years.

Evidence -
1. On the date of inspection (01/25/2022) the following children were without an update to their immunization records: Child #2 (most recent update - 06/03/2021), and Child #4 (most recent update - 02/01/2021).
2. The Provider confirmed this finding on the date of inspection.
Plan of Correction: These parents will be asked for updates to their child's record. Both are due to have their child go to their doctor's office.
Standard 8VAC20-800-960-E
Based on observation, 2 of 2 bottles observed being refrigerated were labeled with the child's full name, but not today's the date.

Evidence -
1. On the date of inspection (01/25/2022), there were a total of (3) infants in care.
2. These two bottles were for Child #4, and Child #8.
Plan of Correction: Bottles will be marked with the name and date on each.
