Inspector Notes

A Subsidy Health and Safety Inspection (SHSI) was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and a virtual tour of the program.

This inspection was initiated and concluded on 01/25/2022. There were 9 children present (27 points) with 3 caregivers supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined (1) non-compliance with an applicable standard, code, or law. The violation was documented on the Violation Notice issued to the Family Day Home (FDH).

Based on review, the department's 3 hour health and safety update course was not completed this year for the applicable staff person.

Evidence -
1. On the date of inspection (01/25/2022), the annual health and safety training was not completed by Staff #1.
2. The Virginia Pre-Service training was completed on 10/19/2018.
Plan of Correction: This course will be taken.
