During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was completed on 07/03/2025. The on-site inspection began at 11:10 am and ended at 12:00 am. The inspector reviewed compliance in the areas listed above. There were 4 children present and 2 caregivers. The point total was 12. The
inspector reviewed 4 children?s records and 2 caregiver records on-site. This inspection included document review, tour of the facility, interviews,and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on
07/14/2025. A POC submitted after this date will not appear on the public website.
Violations
5Child #2 was observed sleeping in an infant bouncer seat inside their crib.
There was a cloth cover on the diapering station.
REPEAT VIOLATION
The emergency evacuation procedures shall be practiced monthly with all caregivers and children in care during all shifts that children are in care.
The emergency evacuation drill for June 2025 was not completed.
REPEAT VIOLATION
Menus. When meals or snacks are provided by the family day home, the menu for the current one-week period shall:1. Be dated; and posted or placed in an area accessible to parents.
The menu for the month of June was posted in the family day home.
Bottles shall be refrigerated and labeled with the child's full name and the date, if more than one infant is in care.
Two sets of bottles were not labeled with the date.
