Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interview, and a virtual tour of the program.

A monitoring inspection was initiated on September 20, 2021 and concluded on September 21, 2021. The provider was contacted by telephone and a virtual inspection was conducted. There were nine children present, ranging in ages from 2 years 9 months to 6 years of age for a total of 15 points, with two caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four child records and three caregiver records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22VAC40-111-90-A
Based on a review of children's records, the provider failed to obtain documentation of immunizations before the child's first day of enrollment.

Evidence:
1. Child #3's immunizations were dated 09/10/2021. First day of attendance for Child #3 was 09/02/2021.
2. The provider verified the date on the immunizations, and the child's first date of enrollment.
Plan of Correction: In the future the provider will not let a child start attending until immunizations are on file.
