Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 05/23/2025 and completed on 05/28/2025. The on-site inspection began at 11:00 AM and ended at 12:30 PM. The inspector reviewed compliance in the areas listed above. There were 12 children present and 2 caregivers; the point total was 21. The inspector reviewed 5 children?s records, and 2 caregiver records on-site. This inspection included document review, a tour of the facility and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
