During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 8/26/25. The on-site inspection began at 9:25am and ended at 1:50pm. The inspector reviewed compliance in the areas listed above. There were 119 children present and 24 staff. The inspector reviewed 10 children?s records and six staff records on-site. This inspection included document review, a tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 9/8/25. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-260-B
After the first license, annual approval from the health department shall be provided.
There was no documentation of an environmental sanitation report for the center in 2025.
Plan of Correction: Dept of Heath License was issued, but additional form was not completed. Health Dept was contacted and they indicated they will come out next week and complete the form.
Standard 8VAC20-780-280-B
Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.
In a toddler classroom, a container of disinfectant wipes were left unlocked by a sink in the diaper changing area.
Plan of Correction: Disinfectant Wipes were immediately stored in appropriate locked cabinet.
Staff was retrained to the safe storage of disinfectant wipes.
Standard 8VAC20-780-560-G
When food is brought from home, the following shall apply: the food container shall be sealed and clearly dated and labeled in a way that identifies the owner.
Four of nine water bottles in a preschool classroom were not dated or labeled to show who the water bottles belonged to.
Plan of Correction: VDOE's nutrition guidelines treat water as a distinct and essential nutrient, not as a
food. A publication from the Virginia Department of Behavioral Health and
Developmental Services (which VDOE has circulated) states that while water is
required for bodily functions, it contains no calories, unlike food. This standard does NOT apply to this situation and we do not agree this is a violation- Water is clearly not defined as food. But, we did add missing labels to the water bottles immediately.

Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain all the required information.
The record for Child #1 (enrolled for two years) did not contain documentation of an annual update, which is required.
