The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. There was no documentation to show that an out of state central registry check was requested for prior to hire for Staff #1(employed for three months) and Staff #2 (employed for four months).
Plan of Correction: The previous document containing list of links for out of state checks has been replaced with an updated list provided by the inspector. Employee #1 had a check from an out of date link that did not meet standards. Both Records required for Employee #1 and Employee #2 were requested and have been received.

The center shall obtain documentation of additional immunizations once every six months for children under the age of two years.

Plan of Correction: Records for Child #1 were from 12/24/24 and 09/17/25. The records were 8 months apart not meeting 6 month requirement. It is our policy to meet the standards as required and we will continue to monitor files for compliance. Updated record has been requested
