Inspector Notes

An unannounced, on-site monitoring inspection was conducted on July 23, 2024. The on-site inspection began at 12:20 pm and ended at 2:10 pm. The inspector reviewed compliance in the areas listed above. There were thirty-six children present and eight staff. The inspector reviewed five children?s records on-site and five staff records electronically on July 23, 2024 and July 24, 2024. This inspection included document review, tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on review, facility did not ensure required out of state background checks are completed when an individual has lived outside the state of Virginia within the past 5 years.

Evidence: The record for staff #2 (start date 4/17/2023) had documentation that they had lived in two other states within the past 5 years prior to beginning employment. There was not a completed criminal history search for one state and a central registry search for the other state.
