The center is required to keep a current list of all known allergies, sensitivities, and dietary restrictions in each room or area where children are present. The list must be dated and kept confidential. The most recent list was dated 2/7/25 and didn't include the dietary restriction for child #4, enrolled for over two years,
Plan of Correction: A restriction and allergy list will be updated with each new restriction as soon as we are notified. This list will always be located inside of each food cabinet in each room.
Standard 8VAC20-780-500-B
Disposable diapers shall be disposed of in a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. Staff in the infant room did not use a foot-operated trash can and had to use their hands to touch the exterior surface of the trash can to dispose of diapers.
Plan of Correction: The extra trash can was removed from the room to prevent any confusion as to which one is for diapers. The child lock removed.
Standard 8VAC20-780-520-C
The center shall obtain written authorization noting any known adverse reactions if diaper cream is used. There was no parent authorization for a diaper cream that was used on two separate dates for child #6.
Plan of Correction: Child #6's written authorization for his diaper cream was filled out and signed same day as inspection. I will begin a daily check on all topical creams in the center to ensure everyone has a signed authorization form.
Standard 8VAC20-780-560-G

1. There were approximately 30 containers of baby food from child #3 that was not labeled or dated in the infant room.
2. None of the food containers in the toddler room and in the 3-4 year old classrooms were dated.
Plan of Correction: Containers of food were labeled with names & dates. Lunchboxes are labeled & dated on reverse side.
