Inspector Notes
The purpose of today?s visit was to monitor compliance during a routine unannounced compliance visit.
Upon my arrival, I was greeted by the provider. There were 7 children and 2 staff. The children were observed individual activities and book reading.
Limited licensing requirements were monitored
The following items were observed in compliance during today?s visit: proper storage of hazardous materials, use of approved space, physical plant, administration, and programs. A total of 2 child records and 1 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

2Evidence: Staff #1 did Tuberculosis Screening expired on 8/05/2020.
Evidence:
1. The provider attendance record was posted on the board. The provider did not record attendance on November 7, 8, 9, and 10.
