Inspector Notes

An unannounced monitoring inspection was conducted today between the hours of 11:25am through 12:25 pm. There were a total of eight children (13 points) observed with three caregivers. A total of four children's files and six staff/household member's files were reviewed. The children were engaged in the following: toy play, games, lunch, arts and crafts, hand washing, and other organized activities. Areas of non-compliance are identified in the violation notice. Based on record review, the provider did not obtain documentation that the child has been adequately immunized.
Evidence:
1. Child #4's (Start date- 7/11/22) record did not contain documentation of the immunization records.
2. Staff #1 confirmed that the immunization records were not in the record. The immunization records were obtained after the inspection took place.
Plan of Correction: The immunization records were obtained after the inspection was completed.
