Inspector Notes

An unannounced monitoring inspection was conducted today between the hours of 9:30 am through 10:45am. There were a total of 12 children (23 points) observed with two caregivers. A total of three children's files and six staff/household member's files were reviewed. The children were engaged in the following: toy play, singing, tv time, snack, bathroom breaks, hand washing, and other organized activities. Standard 22.1-289.035-B-4
Based on record review and interview, the family day home did not conduct an out of state search of child abuse and neglect and sex offender registry check for an individual that lived outside the state Virginia the preceding five years.
Evidence:
1. Staff #3's record (Date of Hire-05/30/2023) contains a Sworn Statement disclosing that they lived in the state of Arizona in the last five years. There was no documentation of an out of state child abuse and neglect check and sex offender registry check.
2. Staff #1 confirmed that an out of state child abuse and neglect registry check and sex offender check was not conducted.
Plan of Correction: An out of state child abuse neglect registry search and sex offender registry check will be conducted.
Standard 8VAC20-770-60-C-2
Based on record review and interview, the family day home did not have a central registry finding within 30 days of employment.
Evidence:
1. Staff #3's record (Date of Hire-05/30/2023) did not contain a Virginia Central Registry finding.
2. Staff #1 confirmed that Staff #3 did not have a central registry finding within 30 days of employment.
Plan of Correction: A VA Central Registry search will be completed.
Standard 8VAC20-800-100-A
Based on record review and interview, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to the child's attendance or within 30 days after the first day of attendance.
Evidence:
1. Child #2's record (Start date-02/01/2023) did not contain documentation of a physical examination.
2. Staff #1 confirmed that documentation of a physical examination was not obtained for Child #2.
Plan of Correction: The parents have been contacted and the parents will provide the provider with a copy of the physical examination.
Standard 8VAC20-800-830-A
Based on documentation review and interview, the emergency evacuation procedures were not practiced monthly with all caregivers and children in care during all shifts that children are in care.
Evidence:
1. Emergency evacuation drills were not conducted for the months of February, March, April, May, and June 2023. Staff #1 confirmed that the emergency evacuation drills were not conducted for the months of February, March, April, May. and June.
2. The Emergency Evacuation Drill log was not completed for the months of February, March, April, May, and June 2023.
Plan of Correction: The provider will conduct emergency evacuation drills for each month. The provider will conduct additional drills to make up for the missed months.
Standard 8VAC20-800-90-A
Based on record review and interview, the provider did not obtain documentation that the child had been adequately immunized.
Evidence:
1. Child #2's record (Start date-02/01/2023) did not contain documentation of immunizations.
2. Staff #1 confirmed that immunizations were not obtained for Child #2.
Plan of Correction: The provider has contacted the parents and the parents will provide the immunization records to the provider.
