Inspector Notes

An unannounced monitoring inspection was conducted on January 4, 2024 between the hours of 9:45 am through 11:30am. There were a total of 11children (24 points) observed with two caregivers. A total of five children's files and five staff/household member's files were reviewed. The children were engaged in the following: toy play, singing, bathroom breaks, hand washing, and other organized activities. Areas of non-compliance are identified in the violation notice. Repeat Violation

Based on record review and interview, the family day home did not obtain a copy of the results of an out of state sex offender registry check and out of state child abuse and neglect registry check for an individual that has resided in another state for the preceding five years.

Evidence:
1. Caregiver #2's record (Date of hire-06/14/2023) has a sworn statement that states that they lived in the state of Arizona in the preceding five years. Caregiver #2's record did not contain a copy of the results of a sex offender registry check and child abuse and neglect for the state of Arizona.
2. Caregiver #1 confirmed that they had attempted to conduct a search, but had trouble locating the correct documentation.
Plan of Correction: The provider will conduct an online sex offender registry check for Caregiver #2 and an Arizona Child Abuse and Neglect check will be conducted.

Repeat Violation

Based on record review, the family day home does not have a central registry finding within 30 days of employment.

Evidence:
1. Caregiver #2's record (Date of hire-06/14/2023) did not contain a central registry finding within 30 days of employment.
Plan of Correction: The provider will use the online portal to submit a Central Registry Check for Caregiver #2.

Based on record review, the provider did not obtain a subsequent Tuberculosis Screening for a household member.
Evidence:
1. Household Member #3's record did not contain results for a subsequent tuberculosis screening. The last tuberculosis screening was conducted on 08/19/2021.
Plan of Correction: The household Member will get a tuberculosis screening.

Based on observation, caregivers did not wash their hands with liquid soap and warm running water after diapering.

Evidence:
1. Caregiver #2 did not wash their hands with liquid soap and warm running water after diapering three children.
Plan of Correction: The caregiver will wash their hands after each diaper change.
Standard 8VAC20-800-680-B
Caregivers did not ensure that children's hands were washed with liquid soap and warm running water after toileting and diapering.

Evidence:
1. Caregiver #2 diapered three children. The children's hands were not washed with liquid soap and warm running water after diapering.
2. Child #1 used the restroom and did not wash their hands after using the toilet.
Plan of Correction: The provider will ensure that all children's hands are washed with soap and warm running water after diapering and toileting.
Standard 8VAC20-800-690-D
Based on observation, the diapering surface was not cleaned and sanitized after use.

Evidence:
1. Caregiver #2 changed three children's diapers consecutively. Caregiver #2 did not clean and sanitize the diapering surface after use.
