Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on July 11, 2025, as a part of the licensure period. The on-site inspection began at 10:05am and ended at 11:05am. The inspector reviewed compliance in the areas listed above. There were nine children present and three caregivers; the point total was 19. The inspector reviewed six children?s records and five caregiver/household member(s) records on-site.
This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-B-3
The family day home must request a search of the central registry prior to the employee's first day of employment.

Staff #3, who has been employed for seven months, did not have a central registry search requested. Staff #3 has never worked alone with the children in care.
Plan of Correction: A central registry search was initiated on 1/6/25 by the family day home, but could not access the new portal. An email for help was sent to OBI, but no response was provided. The provider has emailed OBI for a follow up today to obtain a central registry search for Staff #3.
Standard 8VAC20-800-180-A
A tuberculosis(TB) screening is required every two years from the last screening.

An updated screening for Household Member #2 was not completed and was overdue by five months.
Plan of Correction: The Household Member will get a TB screening today.
Standard 8VAC20-800-90-C
Documentation of additional immunization is required once every six months for children under the age of two years.

Child #3, now 29 months, last documented immunization was 08/21/2024 when Child #3 was 18 months old. There were no current immunizations after this date.
Plan of Correction: The parent will be contacted to obtain current immunization records for Child #3.
