Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on January 23, 2026, as a part of the licensure period. The on-site inspection began at 10:00amand ended at 10:45am. The inspector reviewed compliance in the areas listed above. There were nine children present and two caregivers; the point total was 21. The inspector reviewed six children?s records and five caregiver/household member(s) records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

The record for Child #5, enrolled for three months, did not contain documentation of a physical examination prior to the child's attendance or within 30 days after the first day of attendance.
Plan of Correction: The provider will obtain a copy of the physical examination from the parent and document the record.
Standard 8VAC20-800-60-B
A child's record shall contain required information.

The record for Child #4, enrolled for three months, did not contain the policy number of the child's medical insurance.

The record for Child #5, enrolled for three months, did not contain written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately.
Plan of Correction: The required information will obtained from the parent and the records will be documented.

Repeat Violation

Before a child may attend the family day home, the provider shall obtain documentation that the child has been adequately immunized.

The record for Child #5, enrolled for three months, did not contain documentation that the child has been adequately immunized.
Plan of Correction: The provider will obtain the immunization records from the parent. Moving forward, the provider will ensure to obtain immunization records before the first day of attendance for each child.
