Inspector Notes

This Subsidy Health and Safety Inspection (SHSI) was initiated by licensing staff and concluded on 04/06/2022, between the hours of 10 am to 12:30 pm. There were 4 children present (10 points), with 1 caregiver, and 1 volunteer supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined that there were two (2) non-compliance with applicable standards, code, and/or law. Violations were documented on the Violation Notice issued to the program.

Standard 22VAC40-665-230-A
Based on review, it could not be established that the Provider completed the Virginia Pre-service Training for Child Care Staff.

Evidence -
1. On the date of inspection (04/06/2022), documentation of the completed Virginia Pre-service Training for Staff #1 could not be located.
2. Staff #1 states that this training was obtained and the documentation will be located.
Plan of Correction: This documentation will be located and put in the file for Staff #1.
Standard 22VAC40-665-230-H
Based on review, the department's health and safety update course was not taken on an annual basis.

Evidence -
1. On the date of inspection (04/06/2022), it was determined that the updated health and safety course was not completed in 2021 for Staff #1.
2. Copies of the certificate for the updated course were reviewed for the years of 2018, 2019, and 2020 completed by Staff #1.
Plan of Correction: The updated health and safety course will be completed for 2022 and for every year following.
