During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 09/16/2025. The on-site inspection began at 10 am and ended at 11:30 am. The inspector reviewed compliance in the areas listed above. There were (3) children present and (1) caregiver; the point total was (6). The inspector reviewed (4) children?s records, (1) caregiver record, and (1) household member record on-site on 09/16/2025.
This inspection included: document review (i.e. Files, injury logs, policies and procedures, emergency drill logs, medication - no meds on site); tour of the facility; interviews; and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
2The last documented review of the emergency plan was conducted over 17 months ago.
Within the past year, an evacuation drill (fire drill) was not conducted for each month. Some months had listed only either a lockdown drill or a shelter-in-place drill.
