During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 04/02/26 The on-site inspection began at 8:30 am and ended at 10 am. The inspector reviewed compliance in the areas listed above. CDC: There was (1) child present and (1) staff. The inspector reviewed (5) children?s records and (2) staff records on-site on 04/02/2026.

This inspection included: document review (i.e. Files, injury logs, policies and procedures, emergency drill logs, and medication - no medication), tour of the facility, interviews, and observations.

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards.

Information gathered during the inspection determined (1) noncompliance with an applicable standard. This violation was documented on the violation notice issued to the home.

Children's records are required to contain the name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached.

The record for Child #5 was missing the information for a second emergency contact. While the child was not present during the inspection, Child #5 had been in care for a month.
Plan of Correction: The parent will be contact in order to obtain this information.
