During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be

Inspector Notes

An unannounced, on-site renewal inspection was initiated on 09/26/24 and completed on 10/16/24, as a part of the licensure period. The on-site inspection began at 3:35pm and ended at 5:00pm. The inspector reviewed compliance in the areas listed above. There were 23 children present and three staff. The inspector reviewed five children?s records on-site. Five staff records were reviewed on 09/26/24 at the program's administrative office. Additional documentation was reviewed electronically on 10/16/24. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/14/24. A POC submitted after this date will not appear on the public website.

(REPEAT VIOLATION) Medication shall be labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given.

A prescription medication observed on site for Child #1 was missing the label which contains the child's name, the dosage amount, and the time or times to be given.
Plan of Correction: Per the Center: "Medication was replaced that morning by the mother."
