The central registry search for Staff #1, who has been employed for over six months, was requested over a month after the staff started employment.

The central registry search for Staff #2, who has been employed for over two months, was requested a day after the staff started employment.

Any child for whom emergency medications have been prescribed shall always be in the care of a staff member who meets the requirements in subdivision 1 of 8VAC20-780-245 J.

Child #2 and Child #3 have emergency medications on supply that the center has agreed to administer if needed. The staff present during the inspection were not currently certified in Medication Administration Training (MAT).

The tables where children were observed eating snack were not sanitized before and after they were used.

Staff #2 did not have documentation that two or more references as to character and reputation as well as competency were checked before employment. One reference was obtained one day late, and the second reference was obtained 14 days late.
