Inspector Notes

An unannounced renewal inspection was conducted from 5:30 - 6:00 pm with staff. There were eight children present with two staff supervising. Children were observed washing hands and playing with age-appropriate toys. Two onsite staff records and two onsite child records were reviewed at the time of the inspection along with the daily attendance record, emergency drill log, written list of food allergies, sensitivities and dietary restrictions, the first aid kit, emergency supplies and the required posted information. Three staff records were reviewed off site at the Ron Rosner YMCA on 2/22/2023.

Based on review of three staff records, it was determined that not all repeat Tuberculosis (TB) tests/screenings were obtained at least every two years as required. Evidence: the TB test/screening for Staff #3 had a TB test/screening dated: 1/19/2023.

Based on a review of on-site staff records, it was determined that one record did not include all required information. Evidence: the on-site record for staff #2 did not contain the name, address and phone number of a person to be notified in an emergency.
