Inspector Notes

A monitoring inspection was conducted on 11/19/2021. The inspector reviewed compliance in the areas of administration, staff qualifications and training, physical plant and staffing and supervision. A total of three staff records were reviewed.

The information gathered during the inspection determined one violation with applicable standards or law that is documented on the violation notice issued to the facility.

Based on review of three staff records, it was determined that not all staff had Central Registry Search results within thirty days of employment. Evidence: The record for staff #1 (date of hire: 8/3/2021) did not contain documentation of central registry search results.
