Inspector Notes
An unannounced monitoring inspection was conducted from 2:30 - 3:00 pm with staff. There were 30 children, ranging from 6 years to 11 years of age, present with 3 staff supervising. Children were observed participating in an arts and crafts project. Two children's records, three staff records, the emergency drill log, attendance sheet, written allergy list, and fire and health inspections were reviewed.

Standard 8VAC20-780-160-A
Based on review of three staff records, the faciliy did not have documentation of a tuberculosis (TB) test/ screening for all staff at the time of employment and prior to coming into contact with children. Evidence: the record for staff #2 (date of hire: 3/22/2022) did not have documentation of a TB test/screening and was observed working with children.
Plan of Correction: Staff will be sent to obtain a TB test/screening.
Standard 8VAC20-780-280-B
Based on observation, it was determined that not all hazardous substances were kept in a locked place using a safe locking method that prevents access by children. Evidence: there were multiple bottles of Lysol all purpose cleaner, disinfecting spray and disinfecting wipes in an unlocked cabinet in the storage room. Children were observed walking in and out of the room unsupervised.
Plan of Correction: Going forward, the cabinet with the hazardous substances will always be locked.
Standard 8VAC20-780-340-F
Based on observation, and an interview with staff, it was determined that the facility did not ensure that children under 10 years of age are always within actual sight and sound supervision of staff. Evidence multiple children were observed walking in and out of a storage room, and were out of sight/sound supervision for periods of time that varied from one to 3 minutes. Staff stated that they go in the storage room to retrieve materials.
Plan of Correction: The policy allowing children to go in and out of the storage room has been changed and the storage door will remain closed.
Standard 8VAC20-780-530-C
Based on review of three staff records, it was determined that the facility did not have two staff members with current current First Aid and CPR certification present on the premises during all hours of operation. Evidence: only one staff member had documentation of current First Aid and CPR.
Plan of Correction: The second staff member is signed up to take First Aid and CPR on Saturday, April 9th.
Standard 8VAC20-780-60-A
Based on review of two children's records, it was determined that not all records contained all required information. Evidence: the records for child #1 and child #2, with diagnosed food allergies, did not contain a written care plan to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
Plan of Correction: Staff stated they will ask parents to obtain an allergy care plan from the child's physician.
Standard 8VAC20-780-70
Based on a review of on site documentation, it was determined that not all required information was available for all staff. Evidence: there was no onsite record for staff #1, staff #2, or staff #3 with documentation of a name, address and phone number to be notified in an emergency, or first aid/CPR certification, daily health observation training or Medication Administration Training (MAT) certification.
Plan of Correction: This has been corrected. The binder with staff information was accidently taken off site but it was returned on Monday, April 4th.
