Inspector Notes

An unannounced monitoring inspection was conducted on 11/5/23 from 2:17pm to 3:15pm with program staff. There were 21 children in care, supervised by two staff. Children were observed playing in the gym, using the restroom and washing hands, and eating afternoon snack. Required postings were observed. The attendance, menu, allergy list, and emergency drill log were reviewed. Snack served was yogurt and cheese sticks. Four on-site child records were reviewed. A review of required on-site documentation was conducted for staff certified in CPR/First Aid, Daily Health Observation Training, and Medication Administration Training (MAT). Off-site staff records were reviewed at the YMCA office on 9/6/23. Four medications and authorization forms were reviewed and the center has staff with current certification in Medication Administration Training (MAT). Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 12/4/23. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Standard 8VAC20-780-60-A
REPEAT Based on review of four child records, the program did not obtain all of the required documentation for child records. Evidence: The record for Child C did not contain the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.
Plan of Correction: Review all child files to ensure all information is complete.
Standard 8VAC20-780-80-A
Based on observation, documentation review, and interview, the program did not maintain a written record of daily attendance that documented the arrival and departure of each child in care as it occurred. Evidence: At approximately 2:35pm there were 21 children in care. The written attendance contained documentation of 22 children present. Staff #2 stated that they made a mistake on the attendance but did not know how to fix it. Staff #1 stated that Staff #2 had taken attendance when children arrived but did not sign children out from the morning and did not correctly mark attendance for the afternoon.
Plan of Correction: Review with all staff attendance recording procedures.
