During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, renewal inspection was initiated on 03/03/2025 and completed on 03/04/2025, as a part of the licensure period. The on-site inspection was conducted on 03/03/2025 and began at 7:35am and ended at 8:25am. The inspector reviewed compliance in the areas listed above. There were 32 children present and three staff. The inspector reviewed five children's records on-site. Five staff records were reviewed on 03/03/2025 at the program's administrative office. Additional documentation was reviewed electronically on 03/04/2025. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on
03/26/2025. A POC submitted after this date will not appear on the public website.
Violations
4The TB for Staff #1 was dated 35 days prior to the date of employment.
Staff #1, who had been working with children, completed this course five months late.
The references in the record of Staff #1, who had been employed for 11 months, did not have the dates of contact and one reference did not include the signature of the person making the call.
Child #3 had a serious injury at the center on 01/24/25. On 01/27/25, the center received notification from the parent that the child required outside medical attention because of the injury. As of 03/03/25, the center had not notified the department of the injury.
