An unannounced monitoring inspection was conducted from 3:45 - 4:15 pm with staff. There were 21 school age children, present with three staff supervising. Children were observed playing outside on the playground. Three onsite staff records and two onsite child records were reviewed at the time of the inspection along with the daily attendance record, emergency drill log, written list of food allergies, sensitivities and dietary restrictions, three medications, two accident reports and the required posted information. One staff member on site has current Medication Administration Training (MAT) certification. (Repeat violation) Based on review of documentation, and staff interview, the facility failed to maintain a current written list of children?s allergies, sensitivities and dietary restrictions. Evidence: When asked, staff could not locate an allergy list.

(Repeat violation) Based on review of the written attendance record, and interviews with staff, it was determined that the facility did not ensure that the written record of daily attendance, for each group of children, documents the arrival and departure of all children in care as it occurs. Evidence: there were 21 children observed playing on the playground, and no children were listed as present on the attendance sheet for 4/29/2024.
