During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on 04/15/2025 and completed on 04/16/2025, as a part of the licensure period. The on-site inspection began at 3:50pm and ended at 5:00pm. The inspector reviewed compliance in the areas listed above. There were 19 children
present and two staff. The inspector reviewed five children?s records on-site. Two staff records were reviewed on 04/16/2025 at the program's administrative office. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 05/06/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-200-A
(Repeat Violation) The center shall have a qualified program director or a qualified backup program director who meets one of the director qualifications who shall regularly be on site at least 50% of the center's hours of operation.

There is no documentation that the two staff on site are director qualified. Staff stated that
director qualified staff are not regularly on site 50% of the time.
Plan of Correction: Per the Center: "Director will give 50% of time at the schools."
Standard 8VAC20-780-40-L
(Repeat Violation) The center shall inform all staff who work with children of children's allergies, sensitivities, and dietary restrictions.

During the inspection on 04/15/2025, Staff #2 reported they did not know if any children had any allergies, sensitivities, or dietary restrictions. Per documentation reviewed, a child in attendance had dietary restrictions and food sensitivities.
Plan of Correction: Per the Center: "Placement of allergies are posted in 3 places - staff will have a meeting with any fill in staff"
Standard 8VAC20-780-70
(Repeat Violation) The center should maintain at the center the name, address and telephone number of a person to be notified in an emergency and information about any health problems which may interfere with fulfilling the job responsibilities for each staff member.

Staff #2, who has been employed for over four months, was present at the center during the inspection on 04/15/2025, but the emergency contact information and information about any health problems were not observed at the center for the staff.
Plan of Correction: Per the Center: "Staff that fill in will bring their own certificates as they all receive copies of them."
Standard 8VAC20-780-80-A
(Repeat Violation) For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

When the inspector arrived at the center on 04/15/2025, there were 19 children observed in attendance, but only 17 children listed as present on the attendance sheet.
Plan of Correction: Per the Center: "Staff will record children properly."
