Based on review of five staff records, the program did not obtain the results of a tuberculosis (TB) test within the required timeframe. Evidence: The TB test on record for Staff #4, with an employment date of 10/18/21, was dated 11/29/21. At the time of employment, the TB test for the staff member was due within 21 days of employment.
Plan of Correction: Staff will have TB screening prior to hire date.
Standard 8VAC20-780-240-A
Based on review of five staff records, the program did not ensure that the Virginia Department of Education-sponsored orientation course (Preservice) was completed within 90 calendar days of employment. Evidence: The Preservice certificate on record for Staff #2, with an employment date of 5/20/23, was dated 9/5/22.
Plan of Correction: Make sure all staff complete training within 90 days of hire.
