Inspector Notes

An unannounced monitoring inspection was conducted on 7/10/23 from 8:55am to 10:05AM with the Director and program staff. There were 20 children in care at the start of the inspection, ranging in age from five-years to 13-years-old, supervised by four staff. More children and more staff arrived throughout the inspection. The older group of children were observed transitioning to the playground, coming inside and washing hands, then having morning snack brought from home; younger children were observed eating their morning snack, playing ?sharks and minnows?, then getting their sunscreen to go outside. Five child records and five staff records were reviewed for required on-site documents. Additional documents for staff records were reviewed by the Licensing Inspector on 7/14/23. The program has multiple staff with current certification in CPR and First Aid, as well as staff trained in Daily Health Observation Training. There were no medications on-site but the program has multiple staff current in Medication Administration Training (MAT). Required postings were observed. The attendance and emergency drill log were reviewed. The first aid kit, flashlight, and battery-operated radio were observed. Based on review of five staff records, the program did not obtain the results of a tuberculosis (TB) test within the required timeframe. Evidence: The TB test on record for Staff #4, with an employment date of 10/18/21, was dated 11/29/21. At the time of employment, the TB test for the staff member was due within 21 days of employment.
Plan of Correction: Staff will have TB screening prior to hire date.

Based on review of five staff records, the program did not ensure that the Virginia Department of Education-sponsored orientation course (Preservice) was completed within 90 calendar days of employment. Evidence: The Preservice certificate on record for Staff #2, with an employment date of 5/20/23, was dated 9/5/22.
Plan of Correction: Make sure all staff complete training within 90 days of hire.
