Inspector Notes

An unannounced monitoring inspection was conducted on 04/23/2024 from 12:15 p.m. to 1:40 p.m. There were 29 children present, ranging in ages from four months to three years old with eight staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and four staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-160-A
Based on review of documentation and interview, the center failed to obtain documentation of a tuberculosis (TB) screening from each staff member that was completed within the last 30 days of the first day of employment.
Evidence:
1. Staff #4?s start date was 03/18/2024. The TB screening on file was dated 03/31/2024. Staff #6 confirmed that the TB screening obtained was not dated within 30 days of the first day of employment.
Plan of Correction: Director will ensure TB screenings are completed before start date.
Standard 8VAC20-780-500-A
Based on observation, the center failed to ensure that children and staff?s hands are washed after a diaper change.
Evidence:
1. In the infant room, Staff #5 changed a child?s diaper and did not wash the child?s hands or Staff #5?s hands after the diaper change.
Plan of Correction: Director immediately addressed the proper diapering and handwashing procedures at the end of the day.
Standard 8VAC20-780-550-F
Based on review of documentation and interview, the center failed to practice lockdown procedures at least annually.
1. The date of the last practice of lockdown procedures was 07/05/2022. Staff #6 confirmed that lockdown procedures had not been practiced at least annually.
Plan of Correction: A lockdown drill was scheduled for 4/26/24.
