During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on December 4, 2024 and completed on December 4, 2024. The on-site inspection began at 10:00 a.m. and ended at 12:15 p.m. The inspector reviewed compliance in the areas listed above. There were 26 children present and nine staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews and observation.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on December 18, 2024. A POC submitted after this date will not appear on the public website.

Staff are to complete the VDOE sponsored online orientation course within 90 days of hire. Staff #4, who had been employed for five months, had not completed the course.

Standard 8VAC20-780-40-M
The center is required to keep a current list of all known allergies, sensitivities, and dietary restrictions in each room or area where children are present.
Children were present in room one, two and three and the list of known allergies, sensitivities and dietary restrictions was last updated four months ago. Child #2, who had been in attendance for two months, had a food allergy on file and the allergy was not on the center?s list. Child #3, who had been in attendance for three months, had a food sensitivity on file that was not on the center?s list.
