Inspector Notes
A monitoring inspection was conducted on 11/13/2023 between the hours of approximately 4:15 pm and 5:35 pm. There were 13 children present, ranging in ages from 12 months to 6 years (31 points), with 2 staff supervising. Children were observed while eating the afternoon snack and playing with toys.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. There were no medications on site today. A total of 3 child records and 3 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. 1) On the date of inspection, the most recently issued license (08/04/23 - 08/04/24) was not posted. The license that was posted was dated 02/04/2023 - 08/03/2023.

1) On the date of inspection, there was no attendance record for the day available for review.
Evidence:
1) The record for Child #2 did not contain the following: Name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached; Name and policy number of the child's medical insurance; Written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately; Proof of the child's age and identity; First date of attendance.
Evidence:
1) The menu available for review was not dated for the current one-week period.
2) The menu did not list substituted food - The menu posted listed the afternoon snack as: tortilla chips, apples and water. The afternoon snack provided consisted of animal crackers and water. This substitution was not listed on the menu.
