Inspector Notes

A complaint inspection was conducted on 11/13/23 to investigate a complaint received by the department involving the area of supervision on two incidents occurred on 08/25/2023 and 10/26/2023.
During the investigation, interviews were conducted with staff, documents, and staff/child records were reviewed.

Standard 8VAC20-800-460-E
Based on interview, the Provider did not ensure that children are safe when exposed to animals.
Evidence:
1) An incident that occurred on 08/26/2023 was reported by the parent of Child #2 (3 yrs) in which the child received a scar on their face resulting from the Provider's dog jumping on the child. 2) The Provider stated that the dog had followed one caregiver and Child #2 got close to him and the dog scratched her.
Plan of Correction: The dogs will not be around the children anymore.
Standard 8VAC20-800-560-B
Based on interview, caregivers did not ensure to actively supervise each child during outdoor play to minimize the risk of injury to a child.
Evidence:
1) The parent of Child #1 stated during interview that on 10/26/23, their child (11 mos) received a noticeable bump on the forehead while the children were outside during playtime. According to the parent, the caregiver did not see the incident and did not have information for the parent on how it happened.
2) Upon interview, the Provider stated that they think a ball hit the child because of what other children told them.
3) The parent states that caregivers have been observed in previous times being on their phones while kids are playing.
Plan of Correction: We will ensure better supervision in the playground by placing ourselves in different areas.
Standard 8VAC20-800-570-A
Based on interview, the Provider did not ensure that a caregiver does not exceed points maximus while supervising children.
Evidence:
1) Parent states that on the date of the incident, 10/26/2023, only one caregiver was present with a large group of children. 2) The Provider states that only one caregiver was present while the Provider and a second caregiver were out picking up children from school.
Plan of Correction: Moving forward, we will ensure that caregivers don't exceed points.
Standard 8VAC20-800-840-A
Based on record review and interview, the Provider did not document in the child's record an injury or accident sustained by a child while at the family day home that requires first aid or emergency medical or dental treatment as required.
Evidence:
1) On the date of inspection, an injury report for the incident on 10/26/2023 was not available for review in Child #1's record. An injury report for the incident with Child #2 was also not available for review.
2) The Provider stated that they informed the parents via text message but a formal injury report was not completed to include in the record.
Plan of Correction: Moving forward, I will complete an accident report.
