Inspector Notes
An unannounced, on-site focused inspection was conducted on 09/23/2024, as a part of the conditional licensure period. The inspector reviewed compliance in the areas listed above.
There were 7 children present and 1 caregiver; the point total was 15.
This inspection included document review (i.e. Injury logs, policies and procedures, emergency drill logs, medication administration logs, authorization forms), tour of the facility, and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within 5 business days from today, which will be the close of business on 09/30/2024. A POC submitted after this date will not appear on the public website.
Violations
9The provider shall obtain from each adult household member a current Report of Tuberculosis (Tb) Screening form published by the Virginia Department of Health or a form consistent with it documenting the absence of tuberculosis in a communicable form.
The records for Household Member #2 and Household Member #3 did not contain a Tb screening.
The provider shall obtain for each adult household member a current Report of Tuberculosis Screening form, every two years from the date of the first screening.
The Tb screening form in the record for Household Member #1 (dated 11/30/2021) was overdue by one year and nine months.
The following substances were observed in areas that were accessible to children:
* One spray bottle labeled as "bleach" was observed on a table on the outside area were the children play and sometimes have lunch.
* Various containers with laundry detergent and bleach were observed inside of the laundry room which door was unlocked.
The family day home shall keep a written record of children in attendance each day.
The attendance record for the day was not completed. The last completed attendance available for review was completed two weeks ago on 09/10/2024.
At the time of entrance, six children were playing on the care area and were observed to be by themselves with no caregiver present to supervise. When asked, the children stated that the provider was upstairs. About approximately a minute later, the provider came downstairs to the care area.
Two over-the-counter medication were observed in a plastic Ziploc bag in the refrigerator's door among food.
Two over-the-counter medication bottles were observed in the refrigerator, stored in a Ziploc bag, with no locking method to prevent access by children.
One bottle of sunscreen was observed inside a box in a low shelf of the diaper changing table, making it accessible to children.
When meals or snacks are provided by the family day home, the menu for the current one-week period shall be dated.
The menu available for review was not dated for the current one-week period. It was dated 02/19-23/2024.
