During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site focused inspection was initiated on 12/04/2024 and completed on 12/04/2024, as a part of the licensure period. The on-site inspection began at 11:30am and ended at 12:40pm. The inspector reviewed compliance in the areas listed above. There were 8 children present and 2 caregivers; the point total was 20. The inspector reviewed 2 children?s records and 2 caregivers records on-site.
This inspection included document review and tour of the facility.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 12/12/2024. A POC submitted after this date will not appear on the public website.

12The findings from the most recent inspection, dated 09/23/2024, were not posted in a conspicuous place on the premises.
There was no physical exam records available for Child #1, who was in care during the inspection and had been in care for eight months.
Providers must obtain a current tuberculosis screening from a caregiver at the time of hire and prior to coming into contact with children.
There was no tuberculosis screening for Caregiver 2, who has been caring for children for two days.
On one wall of the care area there was an area that showed mold. The area was on the higher part of the wall.
Potentially poisonous substances, shall be stored away in areas inaccessible to children.
Poisonous substances were observed on different areas of the fdh as follows: *The door to the laundry room, which is adjacent to the care area, was unlocked. Inside the laundry room, on the floor, there was laundry detergent, carpet shampoo solution, fabric softener, three gallons of bleach. * A small can of paint was observed on a table of the infants area, next to the changing table. * A bag of salt pellets was observed on the floor, next to the fridge, under a toy chair.
The family day home shall keep a written record of children in attendance each day.
At approximately 11:30 am, the attendance record for the day has not been completed. There were eight children in attendance at that moment.
The provider shall ensure that a caregiver does not exceed 16 points.
At the time of entrance, the provider was by herself caring for eight children, which totaled 20 points. A second caregiver arrived 20 minutes after.
There was no record for Child #2, who was in care during the inspection and had been in care for eight months.
Medication, including refrigerated medication shall be kept in a locked place using a safe locking method that prevents access by children.
Two syringes with medication were stored in ziploc bags on the fridge door. No locking method was being used to prevent access by children.
There was no documentation of immunizations for Child #1, who was in care during the inspection and had been in care for eight months.
When meals or snacks are provided by the family day home, the menu for the current one-week period shall be dated.
The menu posted was dated for October 2024.
The thermometer in the freezer compartment showed a temperature of 18?F.
