During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced on-site monitoring inspection was initiated and completed on March 25, 2025. The on-site inspection began at 11:29am and ended at 12:25pm. The inspector reviewed compliance in the areas listed above. There were three children present and one caregivers; the point total was six. The inspector reviewed one child?s records and one caregiver's records on-site. This inspection included document review, tour of the facility, interviews, and observations. The information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Children's record are required to contain a written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately and a written authorization if the child is taken off the premises of the family day home. The record for Child #1 was missing the parent's written authorization for emergency medical care and written authorization if the child is taken off the premises of the family day home. The child was present during the inspection and had been in care for over six months.
Plan of Correction: The provider contacted the parent . The parent came to the family day home to sign the authorization forms. The violation was corrected during the inspection.
