Inspector Notes

An in person renewal inspection was conducted today. There were 3 children (5 points) with on caregiver present. Children were observed finishing snack, working on writing numbers and coloring, enjoying free play activity choices.
2 adult and 3 children's records were reviewed. See Violation Notice for missing information. Items reviewed: Fridge/freezer temps, menu, posted documents, First aid/CPR certifications, rabies vaccination for the dog, bathroom items, emergency supplies, and emergency evacuation drills.

Standard 22.1-289.035-A
Based on a review of records, one adult record does not contain documentation of a repeated Central Registry Search (CPS) in the allowable five year time frame.
Evidence:
The results of the CPS search for adult A is dated 7/18/16 (expired). The results of a repeated search are not available.
Plan of Correction: The background search request was mailed to Richmond in early January. I am waiting on the results.
Standard 8VAC20-800-100-A
Based on a review of records, one child's record does not contain documentation of a completed physical within 30 days of enrollment.
Evidence:
1 of 3 records reviewed did not contain a physical. (Child 2: Date of enrollment: 9/13/21)
Plan of Correction: I will ask the parent to provide a physical for the child.

Based on a review of records, the chimney associated with the Pellet stove does not have a current, annual inspection.
Evidence:
The most recent inspection available for review is dated 12/20202.
Plan of Correction: I have already called the company to schedule an inspection.

Based on a review of records, once child's record does not have all required information documented.
Evidence:

*Signature of person viewing and the date of viewing the child's birth certificate (Child 1).

Plan of Correction: I will ask the parent to date the enrollment form, and bring the birth certificate back so I can properly date the form.

Based on a review of records, one child's record does not contain documentation of a current immunization record.
Evidence:
1 of 3 records reviewed contained the initial shot record for the child at 7 months of age. There are no additional shots documented. (Child 2 Date of birth: 2017)
Plan of Correction: I will talk to the parent about getting the shot records.
