Inspector Notes

An in person monitoring inspection was conducted today. There are 4 children present (7 points) with one caregiver.
2 adult and 4 child records were reviewed. See Violation notice for non-compliances. Children were observed playing with a variety of activities, washing hands, eating snack and having a "Letter C" lesson.

Based on a review of documentation, the provider does not have current CPR certification.
Evidence:
The documentation of certification on file is dated as expiring 7/15/22.
Plan of Correction: I have completed the online portion of the course. The hands on part had to be rescheduled for 10/2/22.

Based on a review of documentation, the provider is not current in first aid certification.
Evidence:
The documentation available for first aid certification expired 7/15/22.

Based on observation and provider statement, a written record of attendance has not been maintained,
Evidence:
There was no written record of attendance available at LI arrival. Provider stated she must have forgotten to print one for this week.
Plan of Correction: I will print the attendance sheet and complete it.

Based on a review of records, one child record did not contain documentation nof all required components.
Evidence:
1 of 4 records reviewed did not contain signed authorization to obtain emergency medical treatment. (child 1)
Plan of Correction: I will ask the parent to sign the authorization.

Based on a review of documentation, the Emergency Preparedness Plan (EPP) has not been reviewed annually.
